3v3 Basketball Tournament RELEASE FORM

This must be completed for EACH player on your team in order for them to participate. If the player is NOT 18 it must be signed by a parent or legal guardian.

In consideration of participation in the Impact Summerfest 3v3 Basketball Tournament (“Event”), the undersigned parent or legal guardian acknowledges and agrees to the following:

Assumption of Risk
Participation in basketball involves inherent risks including, but not limited to, falls, contact with other players, and other injuries that may occur during play. I knowingly and voluntarily assume all risks associated with my child’s participation.

Release of Liability
On behalf of myself and my minor child, I release and hold harmless the tournament organizers, sponsors, facility owners, volunteers, referees, and any affiliated organizations from any and all claims, liabilities, damages, or expenses arising out of or related to participation in the Event.

Medical Authorization
I certify that my child is physically able to participate. In the event of injury or emergency, I authorize tournament staff to seek medical treatment if necessary. I understand that I am responsible for any medical costs incurred.

Rules & Conduct
Participants agree to follow all tournament rules and instructions given by officials or staff. The organizers reserve the right to remove any player or team for unsafe or unsportsmanlike conduct.

Photo/Media Release
I grant permission for photographs or video taken during the Event to be used for promotional purposes without compensation.
