Participant/Parent/Legal Guardian: Permission and Release of Liability for Dodgeball Tournament

The dodgeball Tournament is intended to be a fun, charity activity. However, I acknowledge and understand that there are known and unknown risks associated with participation in this Tournament, which may include but are not limited to serious or nonserious personal injuries. I willingly assume the risks associated with participation. I further accept that there are risks that may arise due to the action or inaction or other participants, which I also willingly assume. I agree to comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation. If the Participant is a minor, I make the foregoing acknowledgments, assumptions and agreements on behalf of both myself and the Participant, and hereby give the Participant permission to participate in the Tournament.

I, for myself, the named Participant, my spouse or legal partner, our heirs, assigns, representatives, and next of kin, agree to hold harmless, release, waive and indemnify the Pentecostal Church of God (PCG) & Messenger College, their respective officers, directors, employees, and volunteers, and any other Tournament staff, from any and all injuries, liabilities or damages in case of illness or injury arising from this activity. This includes any and all suits, claims, demands, costs, fees, damages, losses, judgements, fines, amounts paid in settlement and other liabilities and expenses including but not limited to, actual damages, compensatory damages, punitive damages, fines, and legal fees arising from death, disability, personal injury, property damage or theft, or actions of any kind. Additionally, I acknowledge that the PCG routinely promotes activities through various media. This includes, but is not limited to, newsletters, newspapers, brochures, displays, websites and social media. In doing so, the names and photos of Participants may be included without payment or compensation to the Participant or to the Participant’s parent or legal guardian. I am legally competent to understand and complete this waiver, permission and release. The invalidity or unenforceability of any provision of this waiver, permission and release shall not affect the validity or enforceability of any of its other provisions, which shall remain in full force and effect.

I have read this Permission and Release of Liability and understand that my signature below confirms my agreement. By signing below, I certify that I am 18 years of age or older and am authorized to sign on behalf the Participant and all others named. If the Participant is a minor, I certify that I am the Participant’s parent or legal guardian and am authorized to sign on behalf of myself, the Participant, and all others named.
